Congratulations on being selected as a Preferred Patient of the 16 week weight loss program! This agreement will outline the guidelines and protocols of successfully completing the program and maintaining your Preferred Patient status.

By entering into the ReLive Weight Loss Program, you agree to enter into the 16 week weight loss program willingly and with the highest expectation of success following the guidelines outlined below. First and foremost, by signing this agreement you confirm your desire to lose weight and take control of your health and well-being.

Through the ReLive Weight Loss Program, you will have access to the proprietary weight loss system that includes:

• 12 total consults
• A 60 day supply of peptides (i.e. lipotropic drops and appetite control supplements)
• Eating plan and guidelines
• Weigh in with body composition checks
• App access: Includes food journal, weight loss and progress tracking chart + internal messaging and video conferencing with your weight loss coach
• Compassionate and knowledgeable weight loss coaching
• Styku assessment to confirm your progress and achievements performed once every 30 days (i.e. 3 total assessments)

The 16 Week Weight Loss Program is designed to:

• Create a blueprint for weight loss success
• Establish a diet model to maintain weight loss over time
• Enable you to take control of your weight loss goals for greater success
• Provide a lifestyle change for future health benefits and longevity
• Increase energy, improve sleep, reduce inches, improve metabolism, and control appetite

To protect the integrity of the brand and the program, you agree to:

• Clearly define your goals and the result you would like from the program.
• Follow the steps to a successful “before/after photo” and the New Patient Assessment session.
• Agree to and schedule the required follow up appointments for accountability.
• Follow the 16 Week Weight Loss Program guidelines and protocols exactly as outlined.
• Keep accurate records of your daily meals and the 16 week Weight Loss Program formula dosages.
• Prepare your success testimonial and be willing to share it with others.
• We do use email as a tool to communicate during your Weight Loss Journey, unless you opt out of it.

Additionally, you agree to and understand the following:

• You agree and understand that there are a limited number of new patients accepted into this program and that your weight loss coach will review your progress and willingness to successfully complete the program. If at any time, it is determined that you are neither willing nor desire to continue with the program, you will voluntarily relinquish your spot in the program.
• After starting the ReLive Weight Loss Program, you understand that you are responsible for your success and no refund will be offered by the ReLive Weight Loss program.
• It is your responsibility to schedule weekly appointments with your weight loss coach.
• If you run out of peptides early there will be a charge, use a teaspoon on each dose.
• Lipo Drops will last 45 days to 50 days. Appetite Control will last 60 days. (Not 4 Months)
